When you submit your information to join our participant database or inquire about a clinical trial, we may use and share your health information for the following purposes:

• To Evaluate Clinical Trial Eligibility: We review your medical history, health status, and contact information to determine if you meet the strict criteria required to participate in our current or upcoming clinical trials.

• Treatment and Care Coordination: We may use your PHI to coordinate your care, contact you regarding trial updates, or discuss potential research opportunities with our Principal Investigators and medical staff.

• Required by Law: We will disclose your health information when required to do so by federal, state, or local law, or by regulatory bodies overseeing clinical research (such as the U.S. Food and Drug Administration (FDA) or an Institutional Review Board (IRB)).

Your Health Information Rights

As a potential or active research participant, you hold specific rights regarding your protected health information:

• Right to Inspect and Copy: You have the right to review and receive a copy of the health information we maintain about you.

• Right to Amend: If you believe the health information we have on file is incorrect or incomplete, you have the right to request an amendment.

• Right to Restrict Disclosure: You can request that we limit how we use or disclose your PHI for treatment or evaluation.

• Right to Revoke Authorization: If you grant us permission to use your data, you can revoke that authorization at any time by notifying us in writing. Once revoked, we will stop using your information for those purposes, except to the extent that action has already been taken.
